Medical Weight Management Package Terms and Conditions Scope of Services The Medical Weight Management Package includes telehealth based medical evaluation, medication management when clinically indicated, and scheduled follow up visits during the package period. Services are focused on medical weight management and related health considerations. Weight loss outcomes are not guaranteed. Geographic Availability Services are available only to patients physically located in Nevada, Iowa, or Washington, DC at the time of the appointment, in accordance with provider licensure and telehealth regulations. Patients must confirm their location at each visit. Services cannot be provided outside of these jurisdictions. Eligibility Participation is limited to adults who are medically appropriate for weight management treatment as determined during the initial evaluation. Patients with contraindications, active eating disorders, or conditions requiring in person care may be referred elsewhere. Telehealth Services All services are provided via telehealth. Patients are responsible for ensuring adequate technology, internet access, and privacy for appointments. Medications and Labs Package fees do not include the cost of medications, laboratory testing, pharmacy fees, or compounded medications. Insurance coverage for medications is not guaranteed. Prescribing decisions are based on medical necessity and clinical judgment. Appointments and Follow Up Patients are responsible for attending scheduled appointments. Missed or late canceled visits may be forfeited and may not be rescheduled within the package period. Follow up frequency is determined by clinical need and package structure. Payment and Refund Policy Package fees are self pay and due in full prior to initiation of services unless otherwise stated. Fees are non refundable once services have begun. Unused visits do not roll over beyond the package period unless explicitly stated. No Emergency Services This service does not provide emergency or crisis care. Patients experiencing urgent medical or psychiatric symptoms should seek immediate in person care or emergency services. Clinical Decision Making Treatment recommendations, including medication initiation, adjustment, or discontinuation, are based on patient safety and clinical appropriateness.
